The Central Patient Attachment Registry (CPAR) supports patient continuity by recognizing the hard work you have done to develop your panel to deliver comprehensive primary care. CPAR shares this hard work with other providers in the patient’s circle of care.  

CPAR may also be an integral part of the new payment model for family physicians. 

Community Information Integration (CII) ensures timely access to relevant patient health information. This informational continuity is helping physicians deliver better care to Albertans.

The Abundance of Identification (ID) 

Do you ever feel confused about all the IDs? 5 Common IDs explained. 

ID Description  
Practitioner Identification (Prac ID) The nine-digit code assigned to individual practitioners registering with Alberta Health for the first time. 
Facility Billing ID Number The same as the Billing ID used for reporting and billing. 
WDFA The identifier assigned when your facility is approved for Netcare. 
SOA Addressee ID The same as the Prac ID or the ULI for the person or entity designated to receive statements of account. 
SOAssess Addressee ID The ID that identifies the statement of assessment.
